Covid-19 Mask Policy

Live! Casino Pittsburgh remains focused on the safety of our guests and team members.
 
After careful review of new CDC and Pennsylvania Department of Health guidance, and surveying of our guests and team members, guests who are fully vaccinated no longer need to wear face coverings on our property. 
 
However, masking requirements are still in place for those who are not vaccinated, or partially vaccinated.

Win/Loss Statement

Your individual Win/Loss Statements are available for the last three (3) years of your gaming history, and can be quickly downloaded online on your My Live! Rewards Portal.

Please note that ONLY the Win/Loss statement is available online, and not copies of any W-2G or 1099-forms. 

If you need a reprint of a tax form, you will need to complete a manual request form.

If you would like to mail your request, please print, complete, have the form notarized, and mail to:

Request forms can be completed on property at the Live! Rewards desk.  If completed on property, requests are sent to the Corporate Finance Department and will be mailed to the address on the form unless you mark the form for “pick-up”. You will be notified once when the request is complete to pick up at the Live! Rewards Desk.  Request forms, as well as the document themselves, cannot be sent/received via email or fax. Please allow up to 15 business days for completion of your request.
